Behavioral Health Home Svcs. for Children (AMHC): 1-800-244-6431
A holistic service intended to help children and families manage their behavioral health and emotional needs. Services are provided by a team of individuals comprised of a health home coordinator, a clinical team leader, a nurse, and a family support peer specialist. Targeted Case Management is designed to connect families with resources that will help them meet their goals and make sure the needs of the child are met. Case managers meet regularly with children and families in their homes to develop service plans, identify community resources and advocate for needed supports. They support families by attending school meetings, medical appointments, making referrals, and monitoring services as they progress.

A holistic service intended to help adults 18 and older manage their behavioral health and physical health needs. Services are provided by a team of individuals comprised of Health Home Coordinators, a clinical team leader, a nurse, and a peer support specialist. The team provides case management to assist with accessing needed community services and resources, symptom management, wellness management for improved mental health/physical health, life-skills building, coordination with healthcare providers, and support from peers.

Circle of Care Guidebook (from NAMI):
A great mental health resource for friends and caregivers from NAMI (National Alliance on Mental Illness). NAMI has partnered with the National Alliance for Caregiving (NAC) to create “Circle of Care: A Guidebook for Mental Health Caregivers.” This guidebook emerged from the national study on mental health caregiving, “On Pins and Needles: Caregivers of Adults with Mental Illness.” This study’s findings were released in February 2016. It was the first national survey of mental health caregivers conducted in the United States, and it identified numerous challenges faced by these caregivers. Like our other resources for family caregivers, Circle of Care is designed to guide unpaid friends, family and neighbors who care for someone with a mental health condition. Fact sheets included are intended to assist these caregivers with finding help for the specific challenges identified in the On Pins and Needles study.
